Physician Registration. To register for a doctor account, you will create a username and password
for future logins. You will provide us with your name, email address, phone number, physical address,
medical license, gender, date of birth, Driver’s License, NPI number and UPIN number.
Patient Registration. To register for a patient account, you will create a username and password
for
future logins. You will provide us with your name, email address, phone number, and physical address, date
of birth. You may also provide us with your health insurance information and credit or debit card
information. You may also upload medical records to your account that may contain personally identifiable
information.
Make a Purchase. When you purchase a product or service on the Site, you will provide us with your
name, email address, physical address, phone number, and credit card information. We use Authorize.net Inc.
to process payments. Please review Authorize.net Privacy Policy available here.
Doctor Sessions. When you use our telehealth services to connect with a doctor, we will collect and
store video recordings of the appointments.
Contact Us. To contact us, you will provide us with your name, email address, and phone number.
Information as You Navigate Our Site

Services, such as the following:
